LCP - ADVANCED DAY

LEARNING OUTCOMES

The LCP advanced Day builds on the knowledge & skills base introduced at the LCP Foundation day

1. To consolidate understanding of the concept of pathways in the healthcare arena

2. To consolidate understanding of the 3 key sections of the LCP

3. To review understanding of how to complete the LCP

4. To be able to implement the 10 Step Education Programme

5. To be able to implement the LCP Framework into at least 2 clinical areas having already undertaken a Base Review and completed or currently undertaking post pathway analysis.

6. To interpret the data analysis linked with the LCP Framework

7. To be able to compare & contrast data pre & post implementation

8. To be aware of the LCP & Gold Standards Framework (GSF) and preferred Place of Care as frameworks linked to the End of Life National Initiative.

9. To have an introduction to the National Continuous Quality Improvement Programme (CQIP) Model

10. To understand the role of a clinical Facilitator & Clinical Champion.

11. To have an opportunity to contribute to the National Conference
