Transforming End of Life Care in Acute Hospitals

In 2011 the National End of Life Care Programme Team invited all Acute Trusts across England to be part of the Phase1 Transform Programme to improve end of life care within hospitals.

25 acute trusts which were made up of 40+ hospitals, signed up for a year long programme.

In the North West, The Royal Liverpool and Broadgreen University Hospitals NHS Trust and Salford Royal Foundation Trust and Blackpool Teaching Hospitals NHS Foundation Trust were part of this exciting programme.

The programme is based on the 'The route to success - improving quality end of life care in acute hospitals" (2010) and The End of Life Care Strategy (2008) and "How to guide in Acute Hospitals" (2012) documents.
