Project Outcomes

• Version 11 LCP was transferable to this cohort of patients in the last hours / days of life
• Piloted the generic Version 11 LCP in renal units and determined that as expected no changes to the core document were required.
• Determined the need for supportive information
• Guidelines for LCP Drug Prescribing in Advanced Chronic Kidney Disease (below) - endorsed by the DH, The Renal Association and the British Renal Society produced June 2008
