The Hospital Palliative care Team provides specialist palliative care in the acute hospital setting. The Team has an advisory and educational role and also provides direct care to the patients and families requiring specialist palliative care skills.

We are hoping to work with the network nurses to empower and skill the generic workforce to achieve the palliative care approach and further prompt appropriate referrals to the Palliative Care Team for specialist intervention when required.

Our key educational role is to facilitate and empower the network nurse and the generic worker in the palliative care approach.
