The LCP Framework is a continuous quality improvement programme that can transform hospice care of the dying irrespective of diagnosis into other generic healthcare sectors.

The LCP Central Team is working collaboratively with specialist colleagues to demonstrate transferability of the LCP into more specialist healthcare settings.

Some programmes exist as local Phase 1 pilot projects. Others represent a wider collaborative working group at a national level.  Pathway methodology has been utilised to improve the elements of care at the end of life, for example the rapid discharge pathway.
