Rapid Discharge Pathway

Rapid Discharge Pathway
The Liverpool rapid discharge pathway provides a structured and coordinated process to enable a dying patient to go home to die. It is crucial to the success of the framework that specialist palliative care services take a lead role in the use of this pathway
'My brother always said he didn't want to die in hospital, and with the support of this plan of care he was discharged home to me within four hours of our discussion, and he died at home with us the following day. I can't thank you enough for making this happen.'
